Complete "Minor's Information" section

Complete "School Information" section

Obtain Parent/Legal Guardian's signature

Give to employer to fill out "Employer Information" section

Bring completed application to the College & Career Center (Rm. 324) or email Mrs. Chand at rchand@ycusd.org

Once you have submitted your application, it will be completed within 2 school days. Plan accordingly.
