How to Apply
STUDENTS MUST MEET THE FOLLOWING CRITERIA
STUDENTS MUST MEET THE FOLLOWING CRITERIA
- Be at least 11 years of age.
- Be a US citizen or resident alien.
- Meet low-income guidelines OR be a first generation college student (Both parents did not receive a 4-year college degree).
- Download and print one of the two applications above.
- PARENTS: Complete pages 1 & 2 - Don't forget to sign the bottom of pages 1 & 2 with blue or black ink.
- STUDENTS: Complete pages 3 & 4 - Don't forget to sign the bottom of pages 3 & 4 with blue or black ink.
- Attach a copy of pages 1 & 2 of parents 1040 tax return.
- Attach a copy of the student's Social Security card.
- Attach a copy of student's most recent high school transcript.
- Attach a copy of student's updated high school class schedule.
- Provide documentation IF the student is under legal guardianship of someone other than biological parents.
- Mail the completed and signed application with a copy of pages 1 & 2 of parent's 1040 tax return and copy of the student's Social Security card.
